To mitigate some of the risks associated with COVID-19. The Headhunters Wrestling Club

(“HWC”) is adopting new protocols for its return to training. You and your child must understand

and acknowledge that participating in a physical contact combat sport like wrestling is inherently

conducive to infection spread & that there is no “fool proof” strategy that will completely

eliminate the COVID-19 risk, no matter what protocols are in place. You and your child

understand you are freely participating in HWC activities, knowing full well that you may contract

COVID-19 or incur other injuries through your action or inaction or anyone else’s acts or

omissions. You and your child fully understand that the decision to participate in any HWC

activities is yours and yours alone to make & that you assume all liability, damages, injuries,

costs, etc. assuming all risks, thereby accepting full responsibility. You also release from liability

& waive all rights to sue the Headhunters Wrestling Club, their coaches, volunteers, employees,

officers, landlords & any and all agents from any and all claims for negligence, resulting in

COVID-19 exposure/infection, physical or mental injury, illness, damages, death, economic loss,

or any other claim etc. You and your child agree that you have health insurance. You agree to fully adhere to all HWC Covid-19 Protocols and be bound by this document.

You agree to participate in HWC Activities and abide by the following COVID-19 Protocols.

1. You will not participate if you have come in contact with anyone who has COVID-19 or

exhibit any symptoms of COVID-19, including fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea.

2. You agree to submit to contactless temperature screenings upon each entry. A fever

above 99.9 degrees F will exclude you from participation.

3. Participants agree that you will wear a mask upon entry and until on the mats.

4. Spectating parents must wear a mask upon entry and must social distance or wear masks when social distancing is not possible while in the facility.

5. Coaches and athletes only are allowed on the mats.

You are signing this document freely and voluntarily and understand the legal consequences

involved. BE THIS MY ACT AND DEED.
